Soderstrom Skin Institute is conveniently located, has free parking, and walk-in ground floor access for our patients.

We believe that the best patient is an educated patient. We will explain your diagnoses in detail, discuss your options for treatment and set your expectations for the anticipated result. We recognize that each patient’s situation is unique and we ask that you bring a list of your questions and concerns for us to answer.

Please prepare for your appointment by doing the following:

• Complete the necessary Medical History and/or Patient Information forms (see below)
• Bring your Health Insurance Card / Information
• Prepare a list of allergies and all medications you are taking
• Bring copies of records from previous surgeries / related treatments
• If needed, invite a friend or family member to take notes / help with questions
• Please arrive to your appointment 15 minutes early

For new patients or if it has been three years since your last visit, please complete both the Patient Information Form [PDF] and the Medical History Form [PDF].
